ENROLLMENT FORM AND TERMS AND CONDITIONS FOR O.U.T.S. 2012 SEMINARS (PLEASE READ)

Please read the terms and conditions for enrollment below. On the application, you must choose "I agree" to accept the terms and conditions. If you do not accept the terms and conditions, you will not be accepted for the 2012 seminar.
Terms and Conditions of Attendance for the 2012 Oceanside Umpire Training Seminar
The Oceanside Umpire Training Seminar will be held from March 21st through March 30th, 2012, in Cocoa Beach, Florida. All prospective attendees must agree to the following terms and conditions as laid forth in the following paragraphs and bullet points. O.U.T.S., Inc. and its affiliates are not associated with the National Association of Professional Baseball Leagues, or of Major League Baseball, or any of their affiliates or ball clubs. ATTENDEES MUST BE OVER 18 OR OLDER TO APPLY. FINAL DATE FOR REGISTRATION IS MARCH 5th, 2012.
Seminar Report Date: Attendees should plan on being in Cocoa Beach by 7pm, March 21st, 2012. We realize travel arrangements, especially due to weather and sometimes flights from the West Coast arrive late in Florida. If you cannot make it there by 7pm, do not panic, simply call us at 309-363-9995 and let us know of your travel arrangements. We will be breaking camp on the morning of the 30th of March. Last night in hotel is March 29th.
2012 O.U.T.S. PROGRAM ($750.00/includes hotel): March 21st-March 30th, 2012. Package includes instruction, tuition, and fees. It also includes a nine night stay, double-occupancy, in a hotel in beautfiul Cocoa Beach, FL. Hotel will be announced shortly. This also includes one meal per day, camp t-shirt and camp hat, as well as anything else not listed here that we are able to procure for 2012. Transportation to and from airport is provided for those flying in, and transportation to and from sites is also included. Additional items for attendees that are included later will not result in an increase in fee.
Package B, ($500.00/Hotel Paid on Arrival/Commuter Package): Same as Package A, but for those wishing to stay in hotel, allows umpire to pay for hotel room for nine nights upon arrival at O.U.T.S. If you are commuting, you will not owe anything since you will not need hotel.
By choosing Package B, if you are going to stay in our hotel, YOU MUST PROVIDE PAYMENT ON ARRIVAL, OR YOU WILL BE ON YOUR OWN!
Each of these packages includes a $200 dollar deposit, which can be refunded until February 25th, 2012. After this date, that deposit will be considered unrefundable (see cancellation policy below)
Cancellation Policy: Should an attendee choose to cancel after enrolling in O.U.T.S., the following policies will apply:
1.) the final day to cancel with a full refund will be February 25th, 2012. After that date, a non-refundable $200 fee will apply due to the fact that we must book in advance with the hotel by this date. Attendees who apply after this date WILL be subject to this fee should they choose to cancel.
2.) Cancellation can be made by Certified Mail only, so O.U.T.S. may have a record of when your request to cancel has been given.
3.) There will be absolutely NO REFUNDS if an attendee violates the Code of Conduct below, and, in the opinion of the staff, must be removed from the camp.

5.) At no time will any fees associated by credit card/debit card transactions incurred by O.U.T.S. be refunded.
Code of Conduct: While at the Oceanside Umpire Training Seminar, all attendees must remember that they represent the integrity of the game of baseball, and their demeanor and actions should reflect professionalism at all times. Therefore, we have a few simple rules when it comes to conduct while in attendance at our camp:
1.) Attendees are expected to obey the law at all times. Alcohol is not allowed in our hotel, or at any of other O.U.T.S. related facilities. Offsite, attendees are expected to follow all local, state, and federal laws at all times. Any attendee who is arrested while in attendance will be summarily dismissed from O.U.T.S. with no refund.
2.) Theft of any kind will be grounds for immediate dismissal with no refund.
3.) The staff of O.U.T.S. will not tolerate any conduct that is unbecoming of an umpire on a professional level. All we ask is that each attendee understands they are all part of a bigger picture. If the staff of O.U.T.S. has any problem with unprofessional behavior with an attendee, the decision of the chief instructor will be final.
4.) Show up on time and work hard! Those are the two most important lessons you will ever learn as an umpire. Failure to be prompt has cost many professional umpires their careers. If there is a meeting or game posted, you are expected to be there before that time.
